Combination anticoagulant and antiplatelet therapy in atrial fibrillation patients.

چکیده
The question of whether or not combination antithrombotic therapy is beneficial in terms of a reduction in fatal and non-fatal vascular events in atrial fibrillation (AF) is an important question which essentially remains unanswered. Original randomised controlled trials in AF patients examining the efficacy of combination oral anticoagulant (OAC) and antiplatelet therapy for stroke prevention in AF, by comparing either fixed dose (or low intensity international normalized ratio, INR, <1.5) anticoagulation plus aspirin with dose-adjusted warfarin (INR, 2.0-3.0), or dose-adjusted warfarin ± aspirin, have not reported any additional benefit of fixed dose (or low intensity) anticoagulation plus aspirin over dose-adjusted warfarin alone. 1 The Atrial Fibrillation Clopidogrel Trial with Irbesartan for Prevention of Vascular Events (ACTIVE)-W trial also showed that anticoagulation with dose-adjusted warfarin (INR, 2.0-3.0) was superior to the combination of clopidogrel plus aspirin for the prevention of vascular events in patients with AF at high risk of stroke, particularly in those

Effect of Oral Anticoagulant Therapy on Coagulation Activity and Inflammatory Markers in Patients with Atrial Fibrillation Undergoing Ablation: A Randomized Comparison between Dabigatran and Warfarin
Atrial fibrillation (AF) is associated with inflammatory and hypercoagulability state. Previous studies evaluated the safety and efficacy of dabigatran and warfarin in prevention of thrombothic complications. This study was intended to assess the influence of these drugs on hemostatic and inflammatory markers among patient underwent pulmonary vein ablation. A total of 100 patients with AF w...

OBJECTIVES This trial evaluated the efficacy and safety of the combination of antiplatelet and moderate-intensity anticoagulation therapy in patients with atrial fibrillation associated with recognized risk factors or mitral stenosis. BACKGROUND Warfarin was more effective than aspirin in preventing stroke in these patients; combined therapy with low anticoagulant intensity was ineffective. M...

INTRODUCTION AND OBJECTIVES In the NASPEAF (National Study for Prevention of Embolism in Atrial Fibrillation) trial, combination therapy with an anticoagulant and an antiplatelet was more effective than anticoagulation alone in patients with atrial fibrillation. We report long-term follow-up in these patients, including prospective evaluation of different antiplatelet therapies. METHODS This ...
متن کاملNonrheumatic atrial fibrillation. Risk of stroke and role of antithrombotic therapy.
T here has been a long-standing consensus1 that the risk of embolization in patients with atrial fibrillation and rheumatic heart disease, in particular, mitral stenosis, is sufficiently high to justify anticoagulant prophylaxis. However, there has been no consensus in regard to nonrheumatic atrial fibrillation. The recognition that nonrheumatic atrial fibrillation is common and associated with...
